Job Title: LPN Home Health Assistant

LHC Group is seeking a skilled LPN Home Health Assistant to join our team in Clifton, New Jersey.

Job Summary

The LPN Home Health Assistant will assist the Patient Care Manager in coordinating a team of staff to ensure high-quality care to home health patients. This includes ensuring all team staff are skilled and informed, and committed to providing quality care in an efficient manner in accordance with physician's orders and State/Medicare guidelines.

Responsibilities
  • Assist with phone calls from field staff, physician offices, referral sources, patients, and take messages and orders as needed.
  • Monitor the status of incoming faxes and report information to appropriate personnel.
  • Coordinate communication and faxes applicable documents to physician office.
  • Monitor the status of patients in the hospital to ensure adequate care coordination.
  • Assist the Patient Care Manager with managing various software system reports for effective care coordination and utilization management.
  • Perform any required follow-up in response to health check call or tele-monitoring alerts.
  • Prepare patient list and necessary documents for weekly case conferences.
  • Provide direct patient care, as necessary, in accordance to scope of practice, plan or care and physician orders.
  • Complete LHC required learning courses and additional assignments as requested.
Requirements
  • Current LPN licensure in state of practice.
  • Current CPR certification.
  • Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation.
