Job Summary

We are seeking a highly skilled Registered Nurse to join our team at Orono Commons in Orono, ME. As a Care Management RN, you will play a vital role in providing exceptional patient care and collaborating with our interdisciplinary team to develop effective care plans.

Key Responsibilities:
  • Conduct comprehensive nursing assessments and work collaboratively with the nursing team, other disciplines, patients, and families to develop and implement individualized care plans.
  • Establish realistic, measurable short- and long-term patient goals and interventions to minimize rehospitalizations and optimize patient outcomes.
  • Delegate patient care responsibilities to staff and supervise them to ensure that tasks and interventions are implemented effectively.
  • Communicate patient information with assigned staff and between shifts to ensure seamless care delivery.
Requirements:
  • Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
  • CPR Certification is required.
