RN Nurse Case Manager I

Job Summary:
Elevance Health is seeking a highly skilled RN Nurse Case Manager I to join our team. As a key member of our healthcare team, you will be responsible for providing comprehensive care management services to our members with complex and chronic care needs.
Key Responsibilities:
- Assess and develop individualized care plans to address the unique needs and goals of our members.
- Implement and coordinate care plans through collaboration with internal and external resources, including healthcare providers and community organizations.
- Monitor and evaluate the effectiveness of care plans, making adjustments as necessary to ensure optimal outcomes.
- Interface with Medical Directors and Physician Advisors to develop and implement care management treatment plans.
- Provide education and support to members and their families to promote health and wellness.
- Collaborate with other healthcare professionals to ensure seamless care coordination and transitions.
Requirements:
- 3 years of clinical experience, or any combination of education and experience that provides an equivalent background.
- Current, active, unrestricted RN license from the Commonwealth of Virginia.
- Preferred qualifications include previous case management experience, behavioral health experience, and/or experience working with individuals with intellectual/developmental disabilities.

Work Environment:
Elevance Health operates in a Hybrid Workforce Strategy. Associates are required to work at an Elevance Health location at least once per week, and potentially several times per week. Specific requirements and expectations for time onsite will be discussed as part of the hiring process. Candidates must reside within 50 miles or 1-hour commute each way of a relevant Elevance Health location.
