Acute Care Case Manager

Mass General Brigham is seeking a highly skilled Registered Nurse to fill the role of Acute Care Case Manager. This position will be responsible for coordinating post-acute care services for patients admitted to Home Hospital, as well as overseeing the coordination of care for patients identified as high risk for readmission.
Key Responsibilities- Develop and implement discharge plans for patients in collaboration with the patient, family, caregivers, and healthcare team.
- Conduct ongoing assessments of patients' health status to ensure they are receiving the necessary services and care.
- Facilitate safe, effective, and efficient discharge plans, including making facility/agency referrals and determining availability of services.
- Ensure compliance with Federal and State regulations related to discharge planning, as well as CMS regulations and third-party payer requirements.
- Graduate of an accredited school of nursing with a BSN preferred.
- Current licensure as a Registered Nurse in the Commonwealth of Massachusetts.
- Minimum of three years' clinical experience, with prior experience in acute care hospital experience, healthcare reimbursement, quality improvement, and enhanced models of healthcare delivery.
