Key Responsibilities:

Requirements:
  • Active pharmacy license in the state of employment.
  • Immunization certification through an accredited organization.
  • Submission of required information/documents to state PMP administrators.
  • Bachelor of Science in Pharmacy or Pharm. D. degree.
