Job Summary:

We are seeking a skilled Registered Nurse PRN to join our team at Mercy LIFE Valley View, part of Trinity Health PACE. As a Registered Nurse PRN, you will provide high-quality care to seniors in our community, working collaboratively with our interdisciplinary team to deliver comprehensive services and support.

Responsibilities:

  • Provide skilled nursing care and prescribed treatments to participants
  • Participate in interdisciplinary team meetings to develop and implement care plans
  • Facilitate integration of new participants into the PACE program
  • Communicate and counsel participants and families regarding service needs
  • Complete participant nutritional assessments and reassessments as needed

Requirements:

  • Graduate of an accredited Nursing Degree program
  • Current, unencumbered RN license in Pennsylvania
  • BSN preferred
  • Active CPR Certification required
  • Minimum two years of experience with geriatric population
  • Willingness to learn American Sign Language (ASL); ASL proficiency preferred
  • Proficiency in Microsoft Office
