Job Title: Cardiac Rehabilitation & Wellness Patient Navigator

MarinHealth Medical Center is seeking a highly skilled and compassionate Cardiac Rehabilitation & Wellness Patient Navigator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the seamless coordination of patient care and treatment.

Job Summary:

The Cardiac Rehabilitation & Wellness Patient Navigator will function as the primary care coordinator for patients, collaborating with key clinical staff to ensure comprehensive care and treatment. This role requires excellent communication and organizational skills, as well as the ability to work independently in a fast-paced environment.

Responsibilities:

• Educate patients on the purpose of their visit and schedule appointments.
• Oversee the gathering of patient information and records from healthcare providers in preparation for appointments.
• Enter patient demographics, including interpreter services, into the patient scheduling system and contact patients in advance to confirm appointments.
• Ensure subsequent medical information is added to patient charts as necessary.
• Arrange for interpreter services for all appointments and treatments as needed.
• Track consultation, simulation, and new appointments to be made available to patients as needed.
• Coordinate multi-site appointments and schedule ancillary appointments as requested.
• Refer patients to appropriate staff regarding advance directives, community resources, transportation, and insurance issues.
• Facilitate access to medical teams and inform physicians of status changes or obstacles to patient treatment compliance.
• Coordinate the scheduling of ancillary appointments as requested and follow-up appointments, post-treatment.
• Oversee the rescheduling of consultations and follow-up appointments due to physician cancellation.

Requirements:

• High school diploma required; Associate of Arts (AA) or Associate of Science (AS) preferred.
• Experience working in the ambulatory clinic healthcare setting.
• Ability to work independently in a fast-paced environment with minimal supervision.
• Excellent oral and written communication skills.
• Diverse and culturally sensitive customer service skills.
• Ability to manage high stress, chronically ill populations with complicated psycho-social issues.
• Computer skills in Microsoft Word and Excel.

Preferred Qualifications:

• Two-year navigation experience in Cardiology and/or Wellness programs.
• Prior experience leading groups of clinical and non-clinic staff for care coordination.
• Experience working with healthcare payor systems.
