Nurse Practitioner

At Aperion Care Hillside, we are seeking a skilled and compassionate Licensed Practical Nurse LPN to join our team.

The LPN is responsible for providing direct nursing care to our residents, ensuring their physical and emotional well-being. This role requires strong leadership and communication skills, as well as the ability to work effectively with our interdisciplinary team.

• Provide direct nursing care to residents, including administering medications, performing routine charting duties, and maintaining accurate records.
• Supervise nursing assistants and ensure they are following established safety regulations and protocols.
• Develop and maintain a good working rapport with inter-departmental personnel and other departments within the facility.
• Prepare and administer medications as ordered by physicians, and maintain accurate records of medication administration.
• Perform routine charting duties, including documenting resident care and progress.
• Participate in interdisciplinary team meetings to discuss resident care and develop plans to meet their needs.

• Current unencumbered state license as a Licensed Practical Nurse.
• Ability to read, write, speak, and understand the English language.
• Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
• Current CPR certification.
• Experience in Long Term Care is preferred.
