About the Role

The Oncology Navigation Coordinator at Blessing Hospital plays a vital role in guiding cancer patients through their entire care journey. This position requires a registered nurse who can coordinate care across multiple systems, from diagnosis to survivorship.

Key Responsibilities

  • Serve as the primary patient navigator, ensuring seamless coordination of services across the oncology department and other specialties.
  • Coordinate and manage individualized care plans, integrating all aspects of the patient's treatment according to the plan of care.
  • Attend key clinical interactions, including oncology appointments and consultations with other specialists, to ensure continuity and accuracy of the treatment plan.
  • Act as the main point of contact for patients and their families, providing education on the treatment plan and addressing concerns.
  • Collaborate with oncologists, surgeons, radiologists, and other specialists to develop and adjust care plans as needed.
  • Monitor and manage the implementation of the care plan, including scheduling appointments and coordinating tests and procedures.
  • Guide patients in decision-making regarding their treatment options, ensuring they are fully informed and involved in their care.
  • Participate in quality improvement initiatives within the oncology department, focusing on enhancing patient navigation and care coordination.
  • Provide support and resources to patients and families throughout the care process, serving as an advocate for their needs.

Requirements

  • Associate's degree in nursing required; Bachelor's degree in nursing preferred.
  • Oncology Certified Nurse (OCN) certification required.
  • Two years of clinical experience in oncology care required.
  • Current Minnesota RN license and American Heart Association BLS/ACLS certifications required.
