A skilled Registered Nurse is sought to administer high-quality nursing care to patients in their homes. This role involves providing intermittent care in accordance with physician orders and a patient-centered plan of care, under the direction and supervision of the Director of Clinical Services/Nursing Supervisor.
Key Responsibilities:- Deliver nursing services in accordance with the established plan of care.
- Conduct initial evaluations and regular reassessments to determine patient needs.
- Develop and implement individualized plans of care, making necessary revisions as needed.
- Provide specialized nursing services, including preventive and rehabilitative procedures.
- Document patient progress and care conferences in a timely manner, adhering to Agency policies.
- Coordinate services and communicate effectively with patients, families, and healthcare teams.
- Participate in in-service programs and contribute to the development of nursing knowledge and skills.
- Ensure adherence to Agency policies, procedures, and regulatory requirements.
- Process orders, notify physicians of patient needs, and complete certification/recertification orders and discharge summaries.
- Supervise and educate other nursing personnel, as needed.
- Conduct patient care conferences and participate in peer review and performance improvement initiatives.
- Utilize ICD-9 codes accurately and complete OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition within Agency-defined timeframes.
This role involves working in patients' homes, requiring travel to and from patient locations.
Job Relationships:- Supervised by: Director of Clinical Services/Nursing Supervisor
- Workers Supervised: Licensed Practical Nurse, Home Health Aide
This role involves a high level of risk exposure.
Lifting Requirements:The successful candidate must be able to perform the following tasks, if necessary:
- Participate in physical activity.
- Work for extended periods while standing and engaging in physical activity.
- Lift heavy objects.
- Engage in extensive bending, lifting, and standing on a regular basis.
