The Health Services Nurse (LPN) is a vital member of our team at Volunteers of America Mid-States, providing nursing care to individuals with Developmental Disabilities served through the Tennessee Community Services Waiver.
Responsibilities- Implement nursing plans of care by administering medications, nursing treatments, and educating individuals served and staff as directed by the Case Manager - RN.
- Schedule and coordinate medical appointments for individuals served in a timely manner and ensure that all specialist recommendations are addressed.
- Communicate with Primary Care Physicians and specialists to coordinate health services for individuals served.
- Record all nursing care in records and maintain file systems as directed by the Health Services Coordinator.
- Assist in maintaining accurate and up-to-date Health Passports.
- Effectively work to maintain DMRS standards and take prompt, effective action to correct any deficiencies.
- Participate as a member of the Circle of Support for individuals served, reporting any concerns to the Case Manager - RN.
- Be available for shift work and assist in regular household duties.
- Perform other responsibilities as assigned to meet the mission of the agency.
- Participate in performance quality improvement (PQI) duties as assigned by supervision and the PQI committee.
- Be currently licensed as a Licensed Practical Nurse with the State of Tennessee.
- Have experience equivalent to one year of nursing, with experience working with individuals who have Developmental Disabilities and/or Mental Health concerns being desirable.
- Have knowledge of the guidelines established by the state of Tennessee for community support programs for individuals with Developmental Disabilities.
