About the Position

Matrix Medical Network is seeking a skilled clinician to conduct in-home risk assessments for at-risk individuals.

As a Home Risk Assessment Clinician, you will review and observe patients' current health, medical history, medication adherence, social environment, and other risks during a one-hour visit.

This comprehensive assessment provides unparalleled insight into patients' overall health and well-being, helping to identify potential care gaps and improve quality of care.

Key Responsibilities
  • Conduct comprehensive assessments, including medical history, diagnosis, and treatment recommendations.
  • Work collaboratively with physicians, case managers, social workers, and other healthcare professionals as needed.
  • Provide patient education and follow-up care to ensure optimal health outcomes.
  • Offer services in various settings, including home visits, skilled facility visits, and telehealth appointments.
Requirements
  • Masters degree in a relevant field (e.g., nursing, medicine) or commensurate experience.
  • Clinical licensure in good standing, including prescription authority.
  • Board certification in a Matrix-approved specialty.
  • 1+ year of experience as a nurse practitioner or physician assistant.
  • Strong computer skills and familiarity with electronic medical records software.
  • Excellent verbal and written communication skills.
Travel Requirements
  • Able to travel to patients' homes and other locations as needed.
  • Possess a valid driver's license and adequate automobile insurance coverage.
Compensation and Benefits

Earn $2,300 - $11,500 per month based on completed visits and pay rate.

Benefits include mileage reimbursement, 401(k) plan, and limited voluntary benefits.
