Home Health RN Job Description

Interim HealthCare is seeking a dynamic Home Health Registered Nurse to join our team. As a Home Health RN, you will participate in ongoing interdisciplinary assessment of the patient, complete initial and ongoing comprehensive assessments of the patient's needs, and develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).

Key Responsibilities:

  • Participate in ongoing interdisciplinary assessment of the patient.
  • Complete initial and ongoing comprehensive assessments of the patient's needs.
  • Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).
  • Provide services that are ordered by the physician as indicated in the plan of care.
  • Counsel and educate the patient and family in meeting healthcare related needs.
  • Initiate appropriate preventative and rehabilitative nursing procedures.
  • Refer to other services as needed.
  • Monitor and evaluate outcomes of patient care.
  • Communicate with appropriate staff regarding changes in the patient's needs.
  • Supervise paraprofessional staff.
  • Effectively communicate with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care.
  • Communicate with all physicians involved in the plan of care and other health care practitioners related to the current plan of care.
  • Participate in Quality Assessment and Performance Improvement (QAPI) program.
  • Participate in in-services or training, as requested.

Requirements:

  • Graduate of an accredited school of nursing and current RN License in SC or compact license.
  • 2 years' experience as a nurse with 1 year of experience in home health.
  • Proficient computers skills and workplace software knowledge.
  • Work with people, function well under stress, use clinical judgment, be creative and accept challenges.
  • Able to effectively deal with multiple changes based on patient needs and scheduling.
  • Clinical proficiency in nursing, strong communication skills, and compassion.
  • CPR Certification
  • Valid driver's license and proof of insurance
