Your Role: As an Activity Therapist at Aspire Health Partners, you will play a vital role in delivering essential services to our program participants.
Key Responsibilities:
- Facilitating therapeutic interventions in both individual and group settings.
- Developing, updating, and monitoring participant treatment plans.
- Conducting program-specific assessments.
- Documenting treatment details in the Electronic Health Record.
- Maintaining ongoing communication with the program team regarding participant needs and program dynamics.
- Assisting the program team with operational tasks as needed.
- Coordinating appropriate discharge planning and referrals.
- Ensuring compliance through ongoing training and participation in staff meetings.
- Upholding high standards of ethical and professional conduct in all responsibilities.
- Master's Degree in social services or a related field.
- Preferred: 1 year of experience working with the target population.
- Must pass a Level II Background clearance.
