Medical Case Manager Professional

We are seeking a skilled Nurse Case Manager I to join our team at Partnership HealthPlan of California.
About the RoleThis is a unique opportunity to make a meaningful impact in the lives of our members by providing personalized care management services. As a Nurse Case Manager, you will be responsible for coordinating and implementing individualized care plans that address the comprehensive medical, behavioral, and psychosocial needs of our members.
Key Responsibilities- Care Planning and Coordination: Develop and implement individualized care plans that meet the unique needs of each member, including those with complex medical conditions and social determinants of health.
- Case Management Services: Provide case management services independently for a caseload ranging from basic to complex cases, utilizing your knowledge of managed care and experience working with Medicaid populations.
- Collaboration and Communication: Work collaboratively with healthcare providers, community partners, and other stakeholders to ensure seamless coordination of care and effective communication with members and their families.
- Documentation and Record-Keeping: Maintain accurate and timely documentation, records, and case files in our electronic health record system, adhering to NCQA documentation standards.
- Education and Experience: Associate's degree in Nursing required; Bachelor's degree in Nursing (or higher) preferred. 2 years of experience preferred, to include at least one (1) year of case management experience and one (1) year in an acute care setting; or equivalent combination of education and experience.
- Licenses and Certifications: Current and unrestricted California Registered Nurse License. Valid California driver's license and proof of current automobile insurance compliant with PHC policy are required to operate a vehicle and travel for company business.
- Special Skills: Bilingual skills in Spanish, Russian, or Tagalog preferred. Strong organizational, communication, critical thinking skills, and attention to detail required. Ability to work within an interdisciplinary structure and function independently in a fast-paced environment while managing multiple priorities and meeting deadlines.
