About the Role

Responsibilities
  • Conduct comprehensive assessments of patients to develop individualized care plans
  • Provide skilled nursing care to patients in their homes, as prescribed by physicians
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
