Key Responsibilities
  • Conduct thorough biopsychosocial assessments of patients to identify their health care needs and develop effective care plans.
  • Collaborate with healthcare professionals to implement utilization review processes and ensure the most cost-effective use of resources.
  • Communicate with external agencies and payers to facilitate smooth transitions from hospital to post-hospital settings.
  • Identify and initiate discharge planning to ensure timely transitions of patients to post-hospital settings.
  • Assure proper identification of principal and secondary diagnoses in collaboration with treating physicians.
Requirements
  • Current GA RN license
  • BLS certification required within initial 90 days of employment
  • Graduate of an accredited school of nursing
  • Minimum 2 years experience in nursing, preferably in a hospital setting
  • Minimum 1 year experience in case management, utilization management, or discharge planning
Preferred Qualifications
  • Excellent communication, interpersonal, and critical thinking skills
Work Environment

This is a full-time position with a day shift schedule. The ideal candidate will be able to work independently with indirect and periodic supervision.
