We are seeking a highly skilled Clinical Pharmacy Specialist to join our team at ChristianaCare. As a Clinical Pharmacy Specialist, you will play a critical role in providing exceptional patient care and contributing to the development of our oncology and hematology services.
Key Responsibilities- Collaborate with interdisciplinary care teams to maintain and propose best new treatment practice order sets in accordance with NCCN guidelines and primary literature.
- Work collaboratively with IVAS Newark campus in the operational implementation of hematology/oncology order sets and PowerPlans.
- Evaluate drug efficacy, monitor therapeutic goals and outcomes, and review for contraindications/precautions.
- Order and evaluate laboratory tests per approved protocols to support medication therapy monitoring.
- Electronically document interventions and provide sign out as appropriate.
- Assist with projects such as MUEs, drug monographs, and performance improvement activities.
- Review and interpret provider orders and dispense medication.
- Serve as a representative for the Department of Pharmacy on appropriate hospital committees/projects including oncology drug management.
- Navigate oncology drug shortages, including recommendation of alternative therapies when appropriate.
- PharmD degree from an accredited College of Pharmacy required.
- ASHP-accredited PGY1 Pharmacy Practice Residency required or equivalent experience (3 years of clinical pharmacy experience).
- ASHP-accredited PGY2 Oncology Pharmacy Residency required or equivalent experience (3 years of oncology pharmacy experience, in addition to PGY1 residency or equivalent).
- Experience in hospital oncology pharmacy practice preferred.
- Board Certified Oncology Pharmacist (BCOP) required within 2 years of hire.
- Current licensure (or eligible to reciprocate) in the State of Delaware.
