At UnitedHealth Group, we're revolutionizing the way care is delivered. Our Senior Community Care (SCC) program is a pioneering initiative that brings together a multidisciplinary team to provide comprehensive care to patients in their homes, nursing homes, and assisted living facilities.

Key Responsibilities:
  • Conduct thorough assessments to identify patients' needs and develop personalized care plans
  • Manage patients through acute medical situations, chronic condition exacerbations, and short-term rehabilitation episodes
  • Ensure accurate and complete documentation, including ICD-10 conditions
  • Collaborate with the interdisciplinary care team to provide seamless care coordination
  • Conduct advanced illness and advanced care planning conversations with patients and caregivers
Requirements:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted license in the state of practice, or ability to obtain by start date
  • Active Nurse Practitioner certification through a national board (e.g., American Academy of Nurse Practitioners or American Nurses Credentialing Center)
  • Current, active DEA licensure/prescriptive authority, or ability to obtain post-hire, per state regulations
  • 1+ years of NP experience
  • Ability to gain a collaborative practice agreement, if applicable in your state
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions
Preferred Qualifications:
  • 2+ years of NP experience
  • 1+ years of clinical experience in a long-term care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end-of-life discussions
