VNA Health Group is seeking a skilled Registered Nurse to join our Home Health team in Woodbridge, New Jersey.
Key Responsibilities- Assess and evaluate patient needs, developing personalized care plans to promote optimal health outcomes.
- Provide education and support to patients and families, empowering them to manage their care effectively.
- Coordinate referrals to home care services, ensuring seamless transitions and continuity of care.
- Collaborate with multidisciplinary teams to deliver comprehensive care, addressing the unique needs of each patient.
- Document patient progress, updating care plans and outcomes to ensure the highest level of care.
- Current RN licensure in New Jersey
- 2+ years of home health nursing experience
- Excellent communication and interpersonal skills
- Strong organizational and problem-solving abilities
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
