Key Responsibilities
  • Leadership and Management
    • Function in a leadership capacity at the agency, ensuring efficient and seamless service delivery.
    • Manage the CCS Service Delivery Department and their respective staff.
    • Assist in overseeing CCS compliance and coordination.
  • Documentation and Certification
    • Coordinate the completion and compliance of required CCS consumer documentation, including assessment, recovery plan, and billing notes.
    • Ensure completion of CCS required certification, continuing education, and documentation of service facilitation and individual skill work.
  • Collaboration and Communication
    • Coordinate and manage resources with community agencies and other healthcare providers on the clients' team.
    • Collaborate with interdisciplinary teams to optimize patient outcomes.
    • Ability to interact with consumers in a professional, empathetic, and confidential manner.
  • Team Environment
    • Provide a collaborative, productive workplace environment while enriching the team environment.
    • Perform other duties as assigned.
