We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Constellation Health Services. The successful candidate will provide end-of-life care and hospice services to patients in their home or community setting.
The LPN will be responsible for assisting in the planning of patient care under the direction of the RN and the supervision of a Registered Nurse. Key responsibilities include following the plan of care established by the physician, providing treatments, and working collaboratively with the other members of the team to achieve positive patient care outcomes.
The ideal candidate will have a minimum of 1 year of clinical experience and a Connecticut State LPN license. Community home health care/hospice experience is preferred, as well as clinical auditing and wound care experience.
Responsibilities:
- Provides nursing care as defined in State Nurse Practice Act, including specialized duties related to end of life care and/or home care.
- Implements current nursing practice following a comprehensive assessment and the plan of care.
- Maintains accurate and timely documentation of patient services to reflect the plan of care.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Recognize and interpret symptoms and institute remedial measures within the scope and limits of a Licensed Practical Nurse.
- Observes records and reports to immediate supervisor if any changes in patient's condition.
- Participates in coordination of hospice and other home care services, appropriately reporting the identified needs to the interdisciplinary group.
- Participates in IDT meetings.
- Implements care to achieve outcomes of comfortable and safe dying and effective grieving for the patient and family.
- Uses and prepares equipment and supplies effectively, adhering to aseptic technique.
- Provides appropriate pain/symptom management.
- Evaluates the patient's response to treatments/medications.
- Performs other duties as assigned by the registered nurse or clinical supervisor.
