Key Responsibilities- Administer medications, immunizations, and treatment as prescribed by a physician
- Conduct physical assessments of individual health needs and implement appropriate action
- Implement and monitor case management service plans to ensure optimal care and well-being
- Provide education and support to individuals and their families to promote health and independence
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Current licensure as a Registered Nurse in the State of Texas
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
- Excellent communication and interpersonal skills
- Ability to maintain confidentiality and handle sensitive information
