Role Overview
The Clinical Pharmacist for Transitional Care Management plays a crucial role in conducting medication reconciliation after patient discharge through telehealth interactions. This position is vital for patients associated with Oak Street Health and its Risk-Bearing Entity (RBE) during their transition of care. The pharmacist will identify potential medication-related issues and work to reduce the risk of readmission through patient education, therapy recommendations, and coordination of care plans.
Key Responsibilities
- Review discharge summaries and engage with patients, caregivers, providers, and pharmacies to complete medication reconciliation.
- Assess post-discharge medication orders and profiles to prevent interactions and ensure safe medication use.
- Confirm the appropriateness of drug therapies based on patient diagnoses and promote cost-effective medication choices.
- Collaborate with primary care physicians and medical teams to contribute to patient care plans.
- Provide recommendations to the medical team regarding therapies based on clinical evidence and experience.
- Communicate recommendations clearly and effectively, both verbally and in writing.
- Consider patient-specific factors such as age, weight, and organ function when making recommendations.
- Gather patient medical histories and evaluate responses to previous therapies.
- Analyze laboratory data to develop treatment plans and monitor therapeutic responses.
- Utilize pharmacokinetic principles to ensure safe and effective medication regimens.
- Address therapeutic inquiries from healthcare providers.
- Identify and resolve drug-related complications and interactions.
- Establish protocols for detecting significant drug interactions and make recommendations to avoid them.
- Ensure continuity of care by collaborating with other healthcare professionals.
- Educate patients and caregivers on medication administration, potential side effects, and storage.
- Participate in performance improvement initiatives, including problem identification and solution implementation.
- Other duties as assigned.
We seek a motivated and experienced clinical pharmacist with the following qualifications:
- Bachelor of Science in Pharmacy or Pharm.D. Degree required.
- Current and unrestricted Pharmacist license in the state of IN.
- Completion of a PGY1 pharmacy residency plus a minimum of 3 years of experience in an ambulatory healthcare setting, or completion of both a PGY1 and PGY2 ambulatory care pharmacy residency.
- Board Certification (BCACP, BCPS, BCGP, CDCES, etc.) required.
- Ability to calculate medication dosages based on pharmacokinetic parameters.
- Proficient in identifying significant drug interactions.
- US work authorization.
