We are seeking a compassionate and skilled Home Health Nurse to join our team at Shaker for ChristianaCare Direct. As a Private Duty Nurse, you will have the opportunity to work one-on-one with patients in their homes, providing expert care and building meaningful relationships.

Key Responsibilities:
  • Deliver high-quality care to pediatric, adult, and geriatric patients in their homes.
  • Coordinate care with families, healthcare providers, and other stakeholders to ensure seamless care delivery.
  • Administer medications and prescribed treatments as ordered, and provide tracheostomy and ventilator support as needed.
  • Work a minimum of two weekend shifts per month, with flexible scheduling to accommodate patient needs.
Requirements:

To be successful in this role, you will need:

  • A DE LPN or RN license, or equivalent experience in a healthcare setting.
  • Ability to attend a two-week daytime orientation and patient field orientation.
  • Previous home health and/or Long Term Care experience is a plus, with tracheostomy and ventilator experience preferred.
