Your Mission:
  • Provide personalized care to patients, treating in the most suitable setting and advocating for the 'treating in place' model.
  • Conduct thorough assessments, craft tailored care plans, and ensure seamless delivery of Model of Care activities.
  • Play a pivotal role in clinical operations functions, such as prior authorization, medication reviews, and quality gap closure.
  • Foster membership growth and be the cornerstone of communication and coordination between members, their families, and care teams.
Requirements:
  • State unrestricted licensure as an Advanced Practice Nurse, complemented by national board certification.
  • Eligibility to participate in Medicare is a must.
  • A proven track record with at least 6 months of direct experience as an APP, preferably with exposure to geriatric, hospice, or palliative care populations.
  • Experience in the nursing home environment is highly desired.
