Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our medical team at Moab Regional Hospital. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and contributing to the development of evidence-based treatment plans.

Key Responsibilities
  • Collaborate with physicians, care managers, and other clinical staff to develop and implement comprehensive care plans that prioritize patient outcomes and cost-effectiveness.
  • Provide therapeutic assessments and disease management services to patients, working closely with healthcare providers to optimize treatment regimens.
  • Serve as a drug information resource for healthcare providers and support staff, ensuring timely and accurate access to relevant clinical information.
  • Monitor patient response to treatment, identifying potential drug interactions and adverse effects, and making recommendations for adjustments to medication regimens as needed.
  • Participate in quality assurance activities, contributing to the development and implementation of policies and procedures that promote safe and effective medication use.
  • Stay up-to-date with the latest clinical research and guidelines, applying this knowledge to improve patient outcomes and advance the field of pharmacy practice.
Requirements
  • PharmD degree from an accredited College of Pharmacy or equivalent experience and licensure.
  • Minimum 1 year of recent clinical pharmacy experience, with a focus on ambulatory care and formulary development.
  • Strong analytical and problem-solving skills, with the ability to integrate complex information and make informed decisions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with healthcare teams and patients.
  • Proficiency in electronic health records and database information systems, with experience in Microsoft Office Suite.
