Job Summary

Key Responsibilities
  • Deliver high-quality post-acute care to patients
  • Monitor patients' physical and mental well-being, tracking vital signs and documenting health concerns
  • Perform intakes and document medical histories
  • Review medical records and charts
  • Administer and verify medications
  • Maintain medical supplies and contribute to a safe environment
  • Assist with IV set up, vaccinations, and phlebotomies
  • Adhere to care plans and assist with orientation and training
Requirements
  • Current Ohio state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved LPN educational program
  • High school diploma or equivalent
  • Minimum of 6 months of experience in an LPN or related role
  • Ability to lift a minimum of 50 lbs
