We are seeking a skilled and compassionate Registered Nurse (RN) to join our team in Philadelphia and surrounding areas. As a RN Homecare Specialist, you will have the opportunity to work one-on-one with patients, providing high-quality care and making a meaningful difference in their lives.
About the RoleAs a RN Homecare Specialist, you will be responsible for providing skilled nursing care and services to patients in their homes. This includes assessing patient needs, developing and implementing care plans, and evaluating patient progress. You will also be responsible for communicating with patients, families, and healthcare teams to ensure seamless care coordination.
Responsibilities- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Provide skilled nursing care and services in accordance with prescribed orders.
- Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
- Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses.
- Currently licensed as an RN in the state in which the RN will practice.
- Current TB or Chest X-Ray.
- Current BLS CPR card.
