Job Summary:
We are seeking a compassionate Primary Care Physician to join our dedicated team at Oak Street Health. In this role, you will provide exceptional care to Medicare patients in a collaborative and innovative primary care setting.
Key Responsibilities:
- Conduct thorough patient assessments and diagnoses to ensure accurate and effective care.
- Administer primary care services, including prescribing medications and maintaining electronic medical records.
- Collaborate with leadership to achieve health quality goals and improve patient outcomes.
- Manage the center care team to ensure seamless patient care and efficient operations.
- Other duties as assigned to support the success of our team.
Requirements:
- Medical Doctor or Doctor of Osteopathy degree.
- Board Certification in Internal Medicine or Family Medicine.
- Active State License and DEA License.
- US Work Authorization.
- Alignment with Oak Street Health values.
Preferred Qualifications:
- Geriatrics Fellowship training.
- Experience working with geriatric populations.
- Value-Based Healthcare Model experience.
- Outpatient primary care experience.
- Bilingual proficiency is a plus.
