Hospice Nurse Case Manager

Key Responsibilities- Conduct thorough assessments of patients' needs and develop personalized care plans.
- Provide compassionate and skilled nursing care, including patient education and support.
- Maintain open communication with patients, families, and the healthcare team to ensure seamless care coordination.
- Manage patient caseloads and participate in interdisciplinary team meetings to ensure comprehensive care planning.
- Licensed as a Registered Nurse in the state of employment.
- Proficiency in Electronic Medical Records (EMR) software.
- Excellent communication, planning, and organizational skills.
- Ability to work autonomously and make sound decisions.
- Valid Driver's License and reliable vehicle with current insurance.
