We are seeking a skilled Occupational Therapist to join our team at Commonwealth Healthcare at Home. As a Home Health Occupational Therapist, you will play a vital role in helping patients regain their independence and improve their quality of life.

Key Responsibilities:

  • Evaluate patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment.
  • Develop and implement individualized therapy plans to meet patient needs, using evidence-based techniques and in coordination with other healthcare team members.
  • Conduct patient assessments, evaluating functional status and occupational therapy needs, and consult with physicians to develop therapy plans.
  • Observe, record, and report patient responses to treatment and changes in condition to nurse supervisors and physicians.

Requirements:

  • Current Occupational Therapy licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
