Nurse Case Manager

About the Role:
We are seeking a highly skilled Registered Nurse (RN) to join our team as a Case Manager in Home Health. As a key member of our team, you will be responsible for providing high-quality patient care and managing patient cases to ensure optimal outcomes.
Responsibilities:
- Assess patient needs and develop individualized care plans
- Manage patient cases to ensure timely and effective care
- Collaborate with healthcare teams to coordinate patient care
- Provide education and support to patients and families
- Monitor patient progress and adjust care plans as needed
Requirements:
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
- One (1) year experience as a RN
- Valid CPR certification
- Ability to travel to all business locations
