Job Summary

We are seeking a highly skilled and compassionate Continuity of Care Specialist to join our team at Comhar Inc. As a key member of our Long-Term Structured Residences (LTSR) Program, you will play a vital role in helping individuals achieve a more independent and fulfilling life.

Key Responsibilities
  • Coordinate and acquire all necessary services for residents to attain treatment goals.
  • Supervise resident fund management, ensuring entitlements are in place and not jeopardized by under spending or mismanagement.
  • Engage in resident advocacy with internal and external programs and agencies.
  • Assist in ensuring program and facility compliance with internal and external policies, statues, and regulations.
  • Establish relationships with OMH/MR, referring agencies, and external services providers.
  • Coordinate services and follow-ups related to mental health court hearings and commitments.
  • Participate in Quality Improvement Program.
  • Attend internal and external meetings as required.
  • Assist in development and implementation of training programs.
  • Conduct reviews, assessments, evaluations, and safety-related drills as required.
Requirements
  • Bachelor's degree in Human Services or related field required, with at least one year of experience in human services and some quality improvement experience preferred.
  • Associate's degree in Human Services or related field required, with two years of mental health experience and some quality improvement experience preferred.
  • Knowledge of program evaluation, continuous quality improvement, and managed care procedures related to the behavioral health field preferred.
  • Valid PA State Criminal, FBI, and Child Abuse clearances required, with updates every three years per Comhar Inc. policy.
  • Valid driver's license required.
  • Physical examination, including Hep B screening and TB test, required.
  • Proficient with office software.
  • CPR w/AED and First Aid certification required.
