About the RoleThe Clinical Assessment Advisor is responsible for conducting thorough assessments of patient needs, developing and implementing personalized care plans, and coordinating with our interdisciplinary team to ensure seamless care delivery. This role requires strong clinical expertise, excellent communication skills, and a passion for delivering patient-centered care.
Key Responsibilities- Conduct comprehensive assessments of patient needs, identifying areas for improvement and developing targeted care plans.
- Collaborate with our interdisciplinary team to ensure timely and effective care delivery, including coordination with physicians, therapists, and other healthcare professionals.
- Develop and maintain strong relationships with patients, families, and caregivers, ensuring their needs are met and their concerns are addressed.
- Stay up-to-date with the latest clinical guidelines, regulations, and best practices, applying this knowledge to improve patient outcomes and care delivery.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices that enhance patient care and satisfaction.
- Bachelor of Science degree in Nursing (BSN) or related field.
- At least 2 years of clinical experience in home health care, with a minimum of 1 year in a leadership or management role.
- Current CPR certification and valid driver's license.
- Excellent communication, interpersonal, and problem-solving skills.
- Knowledge of state and federal regulations, as well as industry standards and best practices.
- Ability to work independently and as part of a team, with a focus on delivering exceptional patient care and outcomes.
