Caregiver for Individuals with Intellectual Disabilities

Aveanna Healthcare LLC is seeking a qualified Direct Support Professional to join our team. As a Direct Support Professional, you will provide one-on-one services to individuals with intellectual and developmental disabilities in a home and community environment.
Responsibilities- Implement written goals from treatment plans with consumers, including personal care, respite, home supports, and long-term vocational support.
- Instruct and reinforce target goals and outcomes according to consumer needs.
- Complete written documentation of services provided upon completion of visits as directed by the Qualified Professional.
- Accurately note consumer responses and results to staff interventions and assessments through proper documentation.
- Maintain appropriate professional conduct at all times.
- Assist in the development of consumer treatment plans and goal revisions as directed by the interdisciplinary team.
- Provide care in accordance with written instructions from professional staff.
- Report immediately to the Qualified Professional any changes in the patient's condition or any problems in assigned tasks.
- Relay observations of patient's mental and physical status to the Qualified Professional.
- Be dependable and on time for assignments.
- Experience with ID/DD consumers preferred, but not required.
- Proof of eligibility to work in the United States.
- High school diploma or GED.
- Good organizational and communication skills.
- Active driver's license and current automobile insurance.
