Job Summary:

We are seeking a highly skilled Senior Community Outreach Coordinator to join our team at Shakir Consulting Services LLC. As a key member of our staff, you will play a critical role in conducting LTC assessments, creating and reviewing care plans, and coordinating services with individuals served and their interdisciplinary teams. Your primary goal will be to empower individuals to live, work, and attend school within their communities.

Responsibilities:

  • Conduct thorough assessments to understand the needs of our clients and develop tailored care plans that promote independence and community integration.
  • Collaborate with interdisciplinary teams, including healthcare professionals, social workers, and family members, to ensure seamless coordination of services.
  • Connect clients with local resources and support services to achieve their goals and aspirations.
  • Maintain accurate records of client interactions, progress, and outcomes.
  • Participate in meetings on behalf of the agency, providing updates and insights to stakeholders.
  • Mentor new hires and provide guidance on best practices in case management.
  • Foster strong relationships with community partners and organizations to enhance service delivery and outreach.

Requirements:

  • A 4-year degree in Social Work or a related field is required, along with licensure as a Licensed Social Worker by the MN Board of Social Work.
  • No less than one year of full-time experience in social services, preferably working with vulnerable populations.
  • Prior experience in Elderly Waivers, AC, DD, CADI, BI, and CAC is essential.
  • Knowledge of MA, Medicare, and MN Healthcare Programs is highly desirable.
  • Experience working with diverse communities, including African American, East and West African, American-Indian, Hmong, Karen, and Latino populations, is mandatory.
  • A deep understanding of historical racial trauma, cultural responsiveness, and restorative practices is required.
  • A background clearance and reliable vehicle for work purposes are also necessary.
  • Fluency in English, both written and verbal, is expected.
  • A valid driver's license, acceptable driving record, and proof of auto insurance are required.

Preferred Skills:

  • Experience with SSIS, MnSP (MnChoices Support Plan) System, and person-centered thinking and planning would be an asset.
  • Strong oral and written communication skills are essential for success in this role.
  • Excellent time management skills are necessary to meet deadlines and prioritize tasks effectively.

Salary Range:

The estimated salary range for this position is $65,000 - $80,000 per annum, depending on experience and qualifications.
