**Position Purpose:** Develop and coordinate care management activities to provide quality, cost-effective healthcare outcomes. Create personalized care plans and educate members on services and benefit options to improve healthcare access and receive appropriate high-quality care through advocacy and care coordination.
+ Assess member needs, barriers to care, and resources available, recommending and facilitating plans for the best outcome
+ Develop or contribute to personalized care plans and work to identify providers, specialists, and community resources needed for care
+ Provide psychosocial support to members, caregivers, and care managers to access local resources or services such as employment, education, housing, food, independent living, justice, foster care
+ Coordinate between members, family/caregivers, and care provider teams to ensure identified care or services are accessible in a timely manner
+ Monitor progress towards care plan goals and collaborate with healthcare providers for revisions or address identified member needs
+ Collect, document, and maintain all member information and care management activities to ensure compliance with state, federal, and third-party payer regulators
+ May perform on-site visits to assess members' needs and collaborate with providers or resources
+ May educate care managers and/or members on procedures, healthcare provider instructions, care options, referrals, and healthcare benefits
**Salary:** $22.07 - $37.25 per hour
