Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will provide high-quality patient care in post-acute facilities, working closely with our team to ensure the best possible outcomes for our residents.

Responsibilities
  • Provide compassionate nursing care to residents, monitoring their physical and mental conditions and tracking vital signs
  • Perform resident intakes, documenting medical histories and health concerns
  • Administer and verify medications, tracking their use and effects
  • Cultivate a comfortable and safe facility for residents
  • Track residents' medical records and charts
  • Assist with the setup of IVs, immunizations, and phlebotomy procedures
  • Keep track of medical supplies and maintain a proper inventory
  • Be accountable for following and adhering to resident care plans
  • Work with staff in the orientation and training of new colleagues
Qualifications and Skills
  • Current Delaware state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • Degree from an approved LPN training program
  • At least 6 months of experience (LPN or similar role)
  • Ability to lift a minimum of 50 lbs
