Job Summary

PrimeCare Medical, Inc. is seeking a highly skilled Behavioral Health Professional to join our team at the Montgomery County Correctional Facility in Eagleville, PA.

Job Responsibilities:
  • Conduct comprehensive clinical assessments based on referrals and self-referrals, including documentation of findings, mental status examination, case conceptualization, diagnosis, and treatment plan.
  • Make referrals to psychiatry when necessary and conduct risk assessments using structured interviews and risk assessment tools.
  • Manage inmates at risk for suicide and/or engaging in self-harm utilizing existing protocols according to current policy and procedures.
  • Serve as the PrimeCare Medical mental health liaison for mental health services outside the facility.
  • Facilitate admission and placement of inmates/patients to forensic hospitals when necessary.
  • Monitor all mental health services provided and ensure that standards of care are in accordance with NCCHC and accepted psychiatric/psychological standards.
  • Participate in the Quality Assurance Committee and develop and conduct in-service education programs according to the annual in-service calendar.
  • Collect data and prepare monthly reports reflecting performance.
Requirements:
  • Must be licensed as a LPC, LSW, or LCSW in the Commonwealth of Pennsylvania.
  • Must hold a Master's degree or greater in Social Work, Psychology, or related Human Service field with a concentration in clinical course work, training, and/or experience.
  • Minimum of two years of experience.
