Position Summary
The Pharmacist will report directly to the Pharmacy Manager and will be responsible for delivering pharmaceutical care activities for patients within the designated service area. This role is categorized as PRN, requiring flexibility to work onsite during weekdays and occasional weekends for on-call duties.
Key Responsibilities
- Assess and oversee drug therapy in collaboration with healthcare professionals to meet the comprehensive needs of home care patients.
- Ensure compliance with organizational policies, state and federal regulations governing pharmacy practice.
- Participate in quality assurance initiatives and adhere to accreditation standards.
- Facilitate new patient referrals and admissions through effective collaboration with nursing staff and customer service.
- Review and verify all new patient orders, documenting appropriately.
- Assist healthcare professionals in planning and implementing patient care in home settings.
- Establish therapeutic objectives and monitor drug therapy safety and efficacy.
- Conduct thorough assessments to identify potential drug-related issues.
- Evaluate laboratory and clinical data to monitor drug therapy outcomes.
- Provide expert consultation to patients, families, and healthcare professionals regarding medication use.
- Maintain accurate documentation of all clinical activities and communications.
- Process and verify medications and supplies prior to dispensing.
- Stay updated on pharmacotherapy through continuous education and professional development.
Qualifications
- Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
- Active Registered Pharmacist License in relevant states.
- Minimum of two years of experience in an acute-care environment or one year in home infusion therapy.
- Experience in compounding infused medications is preferred.
- Strong organizational and time-management skills.
- Membership in professional organizations is a plus.
- Excellent communication skills, both verbal and written.
- Ability to work flexible hours, including on-call shifts.
