Job Summary
Aveanna Healthcare is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As a Personal Care Specialist, you will provide and document skilled nursing care for patients in their homes, under the supervision of a Registered Nurse.

Responsibilities
As a Personal Care Specialist, your responsibilities will include:
- Delivering and coordinating quality patient care in compliance with physician orders
- Continuously observing and assessing patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate
- Documenting all activities, assessments, nursing actions, responses, and coordination of care in a timely manner whenever care is delivered
- Participating, implementing, and updating the nursing care plan
- Taking appropriate nursing action based on assessment and achieving expected outcomes

Requirements
To be successful as a Personal Care Specialist, you must have:
- A graduate degree from an accredited school of nursing
- A current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures

Preferred Qualifications
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience

Salary
The estimated annual salary for this position is $60,000-$80,000, based on the location in Bangor, PA and the level of experience required.
