- Provide high-quality, patient-centered care to children and adults with chronic conditions in the comfort of their own homes.
- Conduct thorough assessments and develop individualized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Monitor patient progress, report changes to the physician, and make adjustments to the care plan as needed.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements:
- Graduate of an accredited school of professional or licensed practical nursing.
- Current license as an RN or LPN in the state of Michigan.
- Demonstrated ability to read, write, and follow a written care plan.
- Current CPR certification.
- Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair.
- Must have an internet capable device (smart phone, tablet, laptop, etc.) for completion of electronic care documentation.
