Inpatient Rehabilitation Coordinator

Job Summary:
We are seeking a highly skilled Coordinator PAI Care Inpatient Rehabilitation to join our team at Medical City Dallas. This role is responsible for coordinating patient care in accordance with established policies and procedures, ensuring that all patients receive high-quality, efficient care.
Key Responsibilities:
- Assist in transmitting PPS/PAI data to CMS, ensuring regulatory compliance.
- Collaborate with medical staff, patients, and families to coordinate care using a holistic and interdisciplinary approach.
- Assess nursing needs of patients and plan interventions to evaluate outcomes of care.
- Provide direct care within scope and practice, delegating and collaborating with other disciplines and non-licensed staff as needed.
Requirements:
- Basic Cardiac Life Support certification within 30 days of employment start date.
- Current Texas licensure/certification in nursing, physical therapy, speech therapy, or occupational therapy.
- Minimum 3 years recent clinical experience in inpatient rehabilitation, with 1 year experience coordinating PPS and IRF-PAI.
- Pass the licensed personnel FIM exam within 60 days of hire and maintain the facilities UDS-PRO credential.
