POSITION SUMMARY:
The Client Support Specialist is responsible for delivering comprehensive case management services within a nurturing environment for potential and active participants across all programs offered by Entrust Community Services. This role emphasizes positive support and advocacy for individuals and families, collaborating with various agencies and resources to facilitate the development of personalized plans aimed at overcoming barriers.
KEY RESPONSIBILITIES:
· Execute all facets of service delivery, collaborating with the Intake Outreach Case Aid during the assessment process to create and implement tailored service plans for individuals and families.
· Conduct regular reassessments to update service plans, ensuring consistent documentation and communication with participants while tracking their progress.
· Cultivate and maintain supportive relationships with individuals, families, and relevant community agencies involved in the participant's care.
· Distribute finalized plans, reports, and forms to all team members and external agencies, ensuring clarity of assignments related to the service plan.
· Establish and uphold a professional partnership with Intake and Outreach staff and all internal and external stakeholders.
· Facilitate participant exits in accordance with program protocols.
· Collaborate with the Program Manager and service team to identify participants requiring services as openings become available.
· Engage with internal agency personnel and external organizations to ensure proper implementation of all documents and procedures within program databases.
· Investigate community resources that may assist in addressing the support needs of individuals or families engaged in programs.
· Maintain regular communication with internal staff, external agencies, care providers, and families of participants.
· In the absence of yourself or a colleague, coordinate service coverage with team members.
· Additional responsibilities may be assigned as needed.
SUPERVISORY RESPONSIBILITIES:
Reports to: Program Manager
Supervises: None
Purchasing Authority: All supply requisitions, purchase orders, and check requests must be submitted to the supervisor.

EDUCATION AND EXPERIENCE:
Any equivalent combination of education and experience that provides the applicant with the necessary knowledge, skills, and abilities to perform the job. Typically, this includes:
A bachelor’s degree in psychology, sociology, special education, or a related field with one year of experience; or an associate's degree in a related field with two years of experience; or a high school diploma or GED with three or more years of experience in case management, along with additional college-level or specialized human service training.
LANGUAGE AND MATHEMATICAL SKILLS:
Ability to read and interpret documents such as safety rules and procedure manuals. Capable of assisting in the development of training plans for program participants and writing routine reports. Basic math skills such as addition, subtraction, multiplication, and division are also required.
REASONING ABILITY:
Ability to solve practical problems and address various concrete variables in situations with limited standardization. Ability to interpret a variety of instructions in written, oral, diagram, or schedule form.
CERTIFICATIONS:
· Current First Aid/CPR certification preferred
· Valid driver's license required
· Proof of automobile insurance if a vehicle is driven
OTHER REQUIREMENTS:
· Ability to pass a criminal background check
· Ability to pass a mandatory drug screening
· Must not pose a direct threat to the health and safety of self or others
· Ability to pass E-Verify
