Behavioral Health Solutions is seeking a motivated Case Manager and Rehabilitation Coordinator to join our team. As a member of our team, you will have the opportunity to work with individuals living with mental health issues, connecting them to community resources and assisting in skill building.

Key Responsibilities:

  • Develop and implement treatment plans tailored to each participant's needs.
  • Collaborate with treatment providers and staff to ensure effective service delivery.
  • Maintain accurate records and documentation, adhering to relevant regulations and policies.
  • Effectively communicate with participants, their designated supports, and other stakeholders.

Requirements:

  • Bachelor's degree in Health and Human Services or a related field.
  • No more than 2 driving violations within the past 3 years.
  • No DUI or reckless driving within the last 5 years.
  • Pass a Health & Welfare Enhanced criminal history background check and Drug Test.

Salary and Benefits:

  • Pay starts at $20-$24.50 per billable hour, depending on experience and/or credentials.
  • Productivity Incentive for all providers billing over 25 hours a week.
  • A comprehensive benefits package, including health insurance, dental insurance, and employee assistance program.
