Home Health Nurse Care Manager

As a Home Health Nurse Care Manager, you'll play a vital role in our team's success. Your primary responsibility will be to oversee the frequency of visits for our patients, ensuring they receive the highest quality care possible. You'll work closely with our interdisciplinary team to develop and implement comprehensive care plans that cater to each patient's unique needs.
You'll also be responsible for performing accurate OASIS collection, ensuring medication profiles are up-to-date, and communicating timely lab results to our physicians. Your excellent communication skills and ability to work collaboratively with our team will be essential in achieving our goal of delivering exceptional patient care.

**Responsibilities:**
- Oversee the frequency of visits for patients, ensuring timely and efficient delivery of care
- Develop and implement comprehensive care plans in collaboration with our interdisciplinary team
- Perform accurate OASIS collection, medication profile maintenance, and lab result communication
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions
**Qualifications:**
- Current RN licensure in the state of employment
- Minimum one year of clinical experience preferred
- Excellent communication and collaboration skills
- Valid state driver's license and automobile liability insurance
**What We Offer:**
