About the Position:

As a Nurse Practitioner with Matrix Medical Network, you'll conduct comprehensive health assessments in the comfort of our patients' homes. You'll review their medical history, medication adherence, social environment, and other risks to develop a personalized care plan that addresses their unique needs.

Our comprehensive health assessments provide unmatched insight into our patients' overall health and well-being, allowing us to identify potential care gaps and develop targeted interventions to improve their quality of life.

You'll work collaboratively with our team to develop and implement care plans that address the complex needs of our patients. You'll also have the opportunity to participate in ongoing education and training to stay up-to-date on the latest clinical best practices.


Responsibilities:

What to Expect:

As a Nurse Practitioner with Matrix Medical Network, you'll have the opportunity to work in a variety of settings, including home visits, skilled facilities, and telehealth visits. You'll conduct comprehensive health assessments, develop personalized care plans, and work collaboratively with our team to deliver high-quality care to our patients.

Qualifications:

Requirements:

We're seeking a skilled and compassionate Nurse Practitioner with a Master's degree in Nursing and current RN and NP licensure in the state of practice. You'll also need to have a current BLS, ACLS, or CPR certification and a minimum of 1 year of experience as a Nurse Practitioner.

You'll need to have strong computer skills and familiarity with employee health/medical record software. You'll also need to be comfortable and flexible with frequent change and have excellent verbal and written communication skills.

Hours:

We're seeking a Nurse Practitioner who is available to work flexible hours, including days, evenings, nights, and weekends. You'll have the opportunity to work in a variety of settings and will be required to travel to meet patients in their homes.
