About the Role:

Sugar Maple Health is seeking a skilled Physical Therapist to join our Home Care team in the Pasadena, MD area. As a Physical Therapist in this role, you will work with a geriatric population in their independent homes or within senior living/assisted living facilities.

Key Responsibilities:

  • Provide high-quality physical therapy services to patients in their homes or in senior living facilities
  • Conduct thorough assessments and develop personalized treatment plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes
  • Communicate effectively with patients, families, and caregivers to ensure seamless care coordination
  • Stay up-to-date with the latest evidence-based practices and guidelines in geriatric physical therapy

Requirements:
