Key Responsibilities:
  • Ensure that residents' personal care needs are met through regular check-ins and prompt responses to their requests.
  • Deliver care to residents as specified in their care plans, reviewing any updates daily.
  • Communicate any observed changes in residents' conditions to the charge nurse for further evaluation.
  • Assist residents with personal care tasks such as grooming, bathing, oral hygiene, feeding, and toileting as required.
  • Perform assigned tasks by the charge nurse, including taking vital signs and documenting care provided.
  • Safely lift, move, and transfer residents using appropriate techniques and equipment.
  • Document all care and assistance provided to residents in accordance with company policies.
  • Safeguard residents' personal belongings and report any missing items following established procedures.
  • Adhere to infection control protocols, including proper hand hygiene and use of personal protective equipment.
  • Follow fire safety practices and report any unsafe conditions to your supervisor.
  • Complete all required in-service training.
Qualifications:
  • A high school diploma or equivalent experience.
  • Effective communication skills in English with residents, visitors, and staff.
  • Ability to observe, report, and address residents' needs with guidance from supervisors.
  • Experience working with residents in a Skilled Nursing Center is preferred.
